Medical Insurance

  • Multiple plans with Regence Group 3 Administrators (RGA) through BCBS Network
  • Deductible options: $2,000 to $5,000 individual and $6,000 to $10,000 family
  • Preventive care covered at 100 percent
  • FSA, HSA, and HRA options with employer paid contributions available based on plan
  • Telemedicine services included

Kaiser Plans

Available in California, Colorado, and Oregon

  • Four plan options
  • Deductible options: $500 to $3,200 individual and $1,000 to $6,400 family
  • Preventive care covered at 100 percent
  • In network only with required referrals

Dental Insurance

  • Two plans with MetLife
  • Deductible: $50 to $150
  • Annual maximum: $1,500 to $2,000
  • Preventive services covered at 100 percent

Vision Insurance

  • Two plans with MetLife and VSP
  • Copays: $10 to $60
  • $130 allowance for frames or contact lenses

Disability Insurance

  • Short term disability: 60 percent income replacement, up to $1,000 weekly max, 13 day elimination period, up to 180 days
  • Long term disability: 50 percent income replacement, up to $10,000 monthly max, 180 day elimination period

Life and AD&D Insurance

  • Basic group coverage: $25,000 for full time employees
  • Voluntary options: Employee $10,000 to $100,000, Spouse $5,000 to $50,000, Child $2,000 to $10,000

Retirement Benefits

  • 401(k) contributions begin 1 month after employment
  • Employer match on the first 3 percent you contribute after 1,000 hours worked, paid annually
  • Vesting: personal contributions and rollovers are 100 percent vested; employer match is 50 percent vested after year 1 and 100 percent vested after year 2
