Previously known as our Advanced Illness Management (AIM) program, this rebranded and enhanced service reflects our continued commitment to compassionate, patient-centered care that prioritizes comfort, dignity, and quality of life.

Palliative care provides an added layer of support for patients and families, working alongside existing treatment plans and healthcare providers. It is ideal for individuals who are managing complex symptoms and may benefit from expert guidance and holistic care in the home.

Our Three-Part Care Journey

Bristol Palliative Care follows a unique, three-step journey to ensure continuity and quality at every stage:

  1. First Visit
    A comprehensive in-home assessment, including a review of advanced care planning and directives, using the Bristol Palliative Screening Tool to determine eligibility and tailor the care plan.

  2. Interim Visits
    Ongoing follow-up visits focused on symptom management, goal setting, and adapting the care plan to meet the patient’s evolving needs and preferences.

  3. Transition Visit
    When appropriate, a seamless transition to the next level of care—such as hospice—is facilitated, ensuring alignment with the patient’s goals and maintaining continuity of support.
