We are here to help
Bristol Hospice is committed to help you and your family through the whole hospice care process. We know that finding the best hospice care in the area can be a daunting task. Our team is here to help you get the care your loved-one needs.
Bristol Hospice Exceeds National Average
Medicare has asked that every hospice patient receive a complete, comprehensive assessment upon admission; this includes an opportunity to discuss treatment preferences & spiritual beliefs, a complete pain history, and an evaluation of any difficulty breathing. Medicare evaluates the quality of these assessments via an audit called the Hospice Item Set, or HIS. At Bristol Hospice, we have earned a score of 99.5% so far in 2023, far exceeding the national average of 91.3%. Bristol Hospice nurses know that choosing hospice can be a difficult decision and strive to ensure that every patient and every family receive all the care and support they need from day one.

Hospice Eligibility
If you or a loved one have been given a life limiting diagnosis, you may be wondering if it is time to speak to your physician about hospice eligibility. If you fill out the Contact Us form, we will send you an email with a downloadable link to a hospice eligibility checklist. Our checklist does not replace having a conversation with your Primary Care Physician, but it can give you an indication of whether it’s time to have an end-of-life care conversation.
Benefits of Earlier Admission
If you or a loved one have been advised to consider hospice care, we would like to highlight the benefits of starting hospice care from moment you are eligible. If you fill out the Contact Us form, we will send you an email with a downloadable link to a document titled “Benefits of Earlier Admission.” If you are unsure about when is the best time to elect for hospice care, we recommend scheduling an appointment with your Primary Care Physician to discuss end of life care.
